Application for Registration as Insolvency Practitioner

Before completing the registration form below, 

• Certified Copies of Academic Qualifications.
• Proof of Membership to a professional body.
• Evidence of Professional Indemnity.
• Evidence of a bank account for Insolvency matters (i.e Crossed cheque leaf , letter confirming existence of bank account from your bank.
• Passport photo.
• Proof of payment of Application fees of 150,000 USHS (Payment reference number or scan receipt).
